Description
Top-and-bottom(T&B)blood bags have an outlet at the top and at the bottom of thecollecting bag, allowing simultaneous expression of plasma and red cells, whereasthe buffy coat remains in the collecting bag. The composition of bloodcomponents was investigated following manual separation of whole blood in aconventional 4-bag system (A) or automated separation in a ‘top-and-bottom’system (B).
To overcomeinter-donor differences, two units of whole blood were pooled and redistributedinto the original bags (A and B) prior to centrifugation. Leukocyte-poorplatelet concentrates (LPPC) were manufactured from both types of buffy coat (Aand B). The volumes of plasma, red cell concentrates (RCC) and buffy coat weresimilar in both methods. However, the residual leukocytes and platelets in theRCC from the top-and-bottom system were significantly lower than in theconventional system, 140 ± 59 times 10 6 (mean ± SD) versus 762 ± 228 times 106 , respectively (p<0.01). Both types of LPPC contained less than 10 7leukocytes and could be stored for 7 days maintaining a pH above 6.5. Weconclude that the top-and-bottom system enables automated and standardizedpreparation of RCC and plasma with a significantly better buffy-coat removalthan with manual processing.
The quadruple blood bag system has different configuration.
• Sterile, pyrogen free, non toxic , ensure external sterility of the bags,
• Slit present at the bottom of the bag is satisfactory to hang the blood bag during transfusion.
• The plastic blood bag has a shelf life of minimum 2 years.
• Packing of goods: Individual blood bags are packed in a plastic pack and a maximum of 5 bags is packed in aluminum foil pack. These foil packs are packed in the corrugated boxes which indicates clearly and legibly the manufacture name, product name, batch number, quantity, manufacturing and expiry date, gross and net weight and consignee’s name address.
• Collection of blood volume can be 250ml, 350ml, 450ml, 500ml.

DESIGN of Top and bottom quadruple blood transfusion system
• No risk of contamination and airembolism closed system ] with all leak proof seals.
• Slit on both sides of the bags are enough to accommodate 5-10 ml volume test tubes.
• The capacity of the bag is enough to prevent any rupture of the bag from the seam when it is filled to the requisite volume of blood.
TUBING of Top and bottom quadruple blood transfusion system
• Flexible and non kinking
• Non sticking
• Transparent
• Leak proof
• Tubing has same ID/segment number as that on the bag
• The tubes has multiple printed ID/segment numbers
NEEDLE of Top and bottom quadruple blood transfusion system
• 16 gauge short bevel angled
• Siliconized for maximum donor comfort
• Rust proof
• Tightly fixed with hub covered with sterile guard
• Hermetically Sealed
EXTERNAL PORT
• Tamper proof and shouldn’t be recapped
• Easily accessible
PACKAGING
• Protective dual packaging [individual and aluminum] eliminating microbial contamination on surface maintaining thecontents of the bag.
• Easy to handle.
